Rising Stars offers a financial scholarship to those who are in need of financial help to participate in a specific clinic they choose. We determine this by looking at the essay the applicant submits. Also we will look at their or their family’s tax return form to see if the applicant qualifies for the scholarship. To apply for the scholarship please fill out the form below. Applications can be emailed to (risingstar@gmail.com), mailed into (305 N B St, Hamilton, OH 45013), or can be turned into the front desk of the facility.
